Client Referral Resources

Finding resources for clients is hard. Please use these lists to find good resources and ensure they are still active before sending any clients there.

Just sharing these links is not enough to help someone achieve better health and life outcomes! No one database is current or relevant to your client so it is our job as CHWs to get to know specific resources that work best for our communities and refer clients to places where they will receive real help.
